Instructions and Submission Form
Please use this form to request new hardware, new technology, changes to existing hardware or technology, modifications to facilities, purchases, etc., for classrooms and/or labs. Please submit one form for each request. The information gathered will be reviewed and prioritized by a subcommittee of the ITC. The subcommittee will make recommendations to the ITC. The ITC will then forward its recommendations to the CIO and Provost for budgeting considerations. For requests to be considered for the current fiscal year, the deadline is December 1. Materials submitted before December 15th will be processed and forwarded as time permits. This form will remain active and on-going beyond the deadline to allow for requests that could be considered for the next fiscal year.
